The manufacturer shall apply for the Source Approval to SDE (Tech-II), O/o DGM (CACT) Dooravaninagar Bengaluru-16, in the prescribed Application form (CF-801) with all necessary documents as per Data Sheet Form Part-I & II along with two crossed DDs  i.e. one DD of (Rs 5000/- + Service Tax & other levies on ST, as applicable) for Document Study and Documentation (non-refundable) and another for (Rs.17,500 + Service Tax & other levies on ST, as applicable) for Infrastructure Assessment.

On scrutiny of the documents,

a)      If the documents are in order, Infrastructure Assessment shall be carried out by the CACT / QA personnel and samples shall be picked up for the testing.

b)      If the documents are not in order or if IA is not required due to the   

      availability of Valid National /International Approval recognised by CACT,   

      the DD of (Rs. 17500 + Service Tax & other levies on ST, as applicable) received

      for Infrastructure Assessment shall be returned to the Manufacturer by Regd.Post.

The testing shall begin when manufacturer submits a crossed DD towards the CACT testing charges (as mentioned in the Table - 1  + Service Tax & other levies on ST, as applicable) in the prescribed Application form (CF-806).

On successful completion of the tests, Source Approval shall be given.

The manufacturer shall apply for the Source Approval to SDE (Tech-II), O/o DGM (CACT) Dooravaninagar   Bengaluru - 16, in the  prescribed  Application  form ( CF-801 ) with  all necessary   documents  as per Data Sheet Form Part-I  &  II  along with  a crossed DD of (Rs 5000/- + Service Tax & other levies on ST, as applicable) for Document Study and Documentation (non-refundable).

On scrutiny of the documents, if the documents are in order, the manufacturer shall be asked to send the samples from the factory directly to the CACT.

The testing shall begin when manufacturer submits a crossed DD towards the CACT testing charges (as mentioned in the Table – 1 + Service Tax & other levies on ST, as applicable) in the prescribed Application form (CF-806).

On successful completion of the tests, Source Approval shall be given.

The Equipment Manufacturer who is holding the valid Type Approval for supplying the equipments to BSNL / MTNL shall apply for Bill of Materials Approval, for components not approved by CACT during or before the PQT/TSEC, with all necessary documents as mentioned in QM – 324 in the prescribed application form (CF-803), to the SDE (Tech II) O/o DGM CACT along with crossed DD of (Rs 5000/- + Service Tax & other levies on ST, as applicable) non refundable.

If the documents are in order, the manufacturer shall be asked to submit the crossed DD   towards the testing charges for all the components (as mentioned in the Table - 1  + Service Tax & other levies on ST, as applicable) in the prescribed Application form (CF-806)

On successful completion of tests, Provisional Approval for the BOM shall be given.

On scrutiny of the documents, if the documents are in order, samples shall be picked up for testing.

 IN THE SOURCE APPROVAL CERTIFICATE

If the manufacturer requests Change of Name / Change of Address in the Source Approval certificate, the manufacturer shall apply to SDE (Tech-II), O/o DGM (CACT) Dooravaninagar, Bengaluru-16, in the prescribed Application form (CF-804) with necessary document proof such as registration certificate copy, for change of name/ address, along with a crossed DD of Rs. (Rs 5000/- + Service Tax & other levies on ST, as applicable ) for Document Study and Documentation (non-refundable).

In case of change of name only, if the documents are in order, necessary amendment shall be issued.

In case of change of address of the firm or shifting of the factory, the manufacturer has to pay an amount of (Rs.17,500 + Service Tax & other levies on ST, as applicable) towards Infrastructure Assessment, in the form of a crossed DD. Infrastructure Assessment shall be carried out by the CACT/QA personnel, and samples shall be picked up for testing.  The testing shall begin when manufacturer submits a crossed DD towards the CACT testing charges (as mentioned in the Table - 1  + Service Tax & other levies on ST, as applicable) in the prescribed Application form (CF-806).

If the documents are in order, IA/CA report and test reports are satisfactory, necessary amendment with change of address shall be issued.

If the manufacturer has applied for CACT Approval, and the approval process is held up due to delay in submitting Approved Lab. Test report, the manufacturer if required may apply to SDE (Tech-II), O/o DGM (CACT) Dooravaninagar, Bengaluru-16, in the prescribed Application form (CF-810) with the case status, along with a crossed DD drawn in favour of ‘AO (CASH), OFFICE OF CGM QA BSNL’ payable at Bengaluru i.e. DD of (Rs 4500/- + Service Tax & other levies on ST, as applicable ) for administrative charges (non-refundable).

 

Also an undertaking is to be furnished by the manufacturer stating that, in case of any failure observed during testing, they have to replace the supplied items free of cost to supplier under intimation to CACT & QA wing of BSNL.

If found genuine, necessary temporary / extension of approval shall be issued for not more than 3 months pending completion of approval procedures.

Manufacturer may apply to CACT for duplicate copy for the documents like Source Approval Certificate, Extension of Approval / Amendment to Source Approval Certificate, if the Original sent by CACT is got damaged / misplaced / spoiled at their end.   In such cases the manufacturer if required may apply to SDE (Tech-II), O/o DGM - (CACT), Dooravaninagar, Bengaluru - 16, in the prescribed Application form (CF-811) along with a crossed DD drawn in favour of ‘AO (CASH), OFFICE OF CGM QA BSNL’ payable at Bengaluru i.e. DD of (Rs 4500/- + Service Tax & other levies on ST, as applicable) per document, for administrative charges (non-refundable).

On receipt of the DD, necessary duplicate copy shall be issued.
